Avoid at all cost. I have been to 15 SNF'S in greater Los Angeles and this is the worst. Joe McCarthy

I wanted to report an incident that occurred on 7/30/2022 at your facility. My mother-in-law who has Alzheimer's and who had Covid was transferred to your facility for five days. On the fifth day (7/30/2022) we received a call from your front office staff informing us that she had passed away. My wife and her sisters were devastated with this information, within 10 minutes we received another call telling us that we need to move the body. There was no empathy to our mothers passing, there was no information as to how this happened, how long ago, no details. My family was emotionally damaged by this information, and shocked by how insensitive our mothers passing was communicated to us. Regardless, we immediately went to your convalescent home. My sister-in-law was there first and asked to to see her, to her dismay it was not her mom. My mother in law was sitting next to the deceased patient, alive, but certainly not well. She asked how could this have happened to the nurse, the nurse told her that they made a mistake and switched the charts, causing the confusion. I'm severely bothered that the staff are leaving our loved ones, to endure the shame, of laying in their own filth. No one seems to speak English when, when english patients speak. Why can't he have his clothes? And why haven't you given him his paperwork? Cold disgusting slop they call food. A bath? Forget it! They only do those when family calls and demands it; but just for that one time. He is screaming out for help, but no one comes to his aid. I am horrified and I feel so helpless in the matter. I can't get ahold of anyone. I don't know who else to call and they won't allow him to leave smh. I wrote this only as my own experience. I Will never in life recommend anyone that loves their family or friends... Do not send them here. I mean unless you speak Spanish, then you may be fine.

It's a small but pretty Clean Secured Nursing home facility. The staff is nice & tries to accommodate my uncles needs.. It has a small garden which does not suit my Uncle taste, who suffers from Dementia & blood clots. He's used to being out and about and working out so hes unable to do this here & do things for himself or remember to take his meds and it's kind of frustrating for him not to be out in a nice green garden like his old home. But they are able to make him as comfortable as possible.. I wish I could invest & help them make this facility more garden like. We came to visit him before the COVID19 hit. We pan on visiting him again soon

I have had occasion to interact with many convalescent centers due to my mom's physical and mental issues, and the Santa Monica Conv. Center has definitely been the best. I am so grateful for the doctor who was able to get my mom into this center. There is much light in this center, which my mom has always loved, the food is good (well ground for my toothless mom :-) ) and the nurses are very sweet to her. I would recommend this center to anyone who cares deeply for the care and welfare of their loved ones...
